Our Mission
At Otter Tail Health, our mission is to support individuals in achieving stability and independence through in-home and community-based services. We are committed to a person-centered approach that empowers individuals to take control of how their services are delivered, fostering greater autonomy and self-sufficiency.
We are contracted with DHS to provide SSI benefits application with Certified SOAR trained staff.
Our Services
Housing Stabilization Services: We assist with finding, applying for, and moving into your new home, ensuring a smooth transition to stable housing.
Independent Living Support Services: We provide support for daily activities such as hygiene, mobility, personal care, and transportation, helping you maintain independence with Medical Assistance coverage. Our team offers various supports, including night supervision, respite care, companionship, and 24/7 emergency assistance to meet your unique needs.
Other Services: We are also a social security advocate and help our clients confidently navigate the application process of filing for Social Security Disability.
Admissions
Housing Stabilization: To qualify for Housing Stabilization Services, individuals must be over 18 years old, have Medical Assistance, and be experiencing housing instability. This may include those who need to relocate, are transitioning out of an institutional setting, or are currently unsheltered.
- Independent Living support Services: Must have a completed MNChoice assessment.
- Waiver Services: Must qualify under the CADI Waiver.
- Social Security Advocacy Services: Must be 18 or older
This ensures comprehensive support across various needs for eligible individuals.
