We are committed to empowering individuals aged 18 and older, particularly those with developmental disabilities or related conditions, by offering training and support that promotes independent living, helps achieve personal goals, and fosters self-directed lives. We can also provide training to family members as needed.
Relocation Services targeted case management services is a specialized service aimed at supporting individuals residing in qualified institutions who aspire to transition into the community and lead successful and independent lives. Our goal is to facilitate a smooth, well-planned transition by connecting individuals with essential services like medical, social, educational, housing and resources specific to their unique needs and circumstances. Individuals will also access wrap-around services through care coordination to ensure long term stability.
We provide thorough assessments and periodic reviews of completed care plans to ensure they remain aligned with each client's needs and goals. Our team actively collaborates with other service providers to coordinate care and monitor progress, ensuring cost-effective and efficient achievement of the care plan’s objectives.
As Social Security Advocates, we also support clients in applying for Social Security Disability benefits, guiding them through the application process to help secure the assistance.
Through our SSI Advocacy Programs, certified case managers can help individuals confidentially navigate the application process of filing for Social Security Disability. The case manager will collect medical records, fill out all forms, write necessary reports, and submit your initial applications. If a denial is received our caseworkers are able to submit a reconsideration request for the client.
Our case managers are professionally certified by the Substance Abuse and Mental Health Services Administration (SAMHSA) to serve children and adults.
